Student Incident Reports and Insurance Claims
Student Incident Reports
Student Incident Report Forms provide a brief, yet concise description of student injuries/accidents during school hours or while a student is participating in a District/School Board sponsored and supervised activity. If a student injury or accident occurs, the appropriate person at the student's school (principal, school nurse, teacher, coach, etc.) should complete a Student Incident Report Form and fax it to Jason Richie, Jr. at the Perry County School District Central Offices at 439-2512 as soon as possible.
It is important for District records that all information on the form be completed, signed, and dated.
Student Insurance Claims
The Perry County Board of Education provides accidental insurance coverage for all students attending the Perry County Public School District. Accidents which occur during regular school hours, as well as District/School Board sponsored and supervised activities, including interscholastic sports are covered.
It is important to note that coverage from this insurance may start only after the parent's/guardian's/student's personal health insurance has covered all that it will cover.
If a student has an accident or injury from an incident covered by the Perry County Public School District's accident insurance, the following steps need to be taken:
1. A Student Incident Report Form should be completed by the appropriate person at the student's school and faxed to Jason Richie, Jr., at the Perry County School District Central Offices at 439-2512.
2. A Insurance Claim Form should be obtained from the student's school and completed by the student's school and parent/guardian.
3. The parent or guardian should mail the original completed Insurance Claim Form along with any medical bills to the address indicated on the Insurance Claim Form.
4. It would be a good idea for the parent/guardian to keep copies of any claim forms and medical bills/information.
